What Information Should NOT be in an Event?

Personal views, feelings, or opinions of the author or writer should never be part of the case documentation. Stick to facts. Information regarding other individuals receiving services is another example of what should not be included in an event.”

Important Note:

  • If a caller or referent requests their name remain anonymous, you should advise that you cannot guarantee anonymity.
  • This information would only be shared as necessary with law enforcement, Adult Protective Services, and other regulating agencies. This should also be explained to the caller.
  • If the caller reveals their name, it should be documented in KYGFIS.
