There are 4 classifications based on the needs of each individual:

  • High – 1 visit every 30 days
  • Moderate – 1 visit every 90 days
  • Low – 1 visit every 180 days
  • Maintenance – 1 visit every 365 days

*These are the minimum frequencies for face to face visits with an individual. More frequent visits are recommended, and at times may be required.
