Face to Face- Before the Visit

Before the visit:

Review the individual’s information in KYGFIS:

  • Face sheet and alert box to become familiar with the individual and their living environment.
  • Personal information: enjoyed activities, who individual resides with, appearance, past events/incidents, etc.
  • Medical information: diagnoses, Heath & Physical (H&P), caregiver/facility staff information, medications and refill status, etc.
  • Budget and available resources.
  • Know and understand restrictions the individual may have. Be ready to speak with caregiver/staff

Schedule according to the individual’s activities:

  • To obtain a complete picture of an individual’s care, both announced and unannounced visits are required.
  • Staff’s work schedule may have to be adjusted to accommodate face to face visits.
  • Keep in mind, it is recommended that a face to face visit should not occur at an individual’s employment. That may cause undue stress for the individual and an uncomfortable situation for the employer.

Make a plan:

  • Familiarize yourself with the location, parking and layout of the area.
  • Review the Guardianship Face to Face Visit Report Form. While the form is not required to be completed and entered into KYGFIS, the questions contained are required to be addressed and documented during each visit.
