Three ways we can embed a course evaluation.
- Add a button or link to an external evaluation. This evaluation might be hosted in an online software like Qualtrics. See example below.

2. Add an embedded gravity form. Gravity forms are an plugin to this wordpress site. You have to ask permission to access the Gravity Forms plugin. See sample below, which does include some logic (choose yes).
3. Email after the course is completed. I like to use this option when there is a lot going on at the end of the course, like completing a graded assignment. A notification can be sent to learners X number of days after the successful completion of the course. In the example below, this email is sent 1 day after the learner completes the course. It contains a link to the evaluation, and also to the most important resources in the course.
