Face to Face- During the Visit

During the visit:

Travel Light:

  • Take only what is necessary into the home/facility: pen, notepad, state-issued cell phone, vehicle keys.
  • Some facilities do not allow items in certain areas. Some items may be required to be placed in a locker or left in the vehicle.

Observe:

  • How is the individual interacting with their surroundings? Staff? Co-residents?
  • Does the individual appear well taken care of (clean, groomed, odor)? How are they emotionally/mood?
  • Is the facility in good condition? Any concerns about the environment? 

Engage:

  • What do they enjoy about where they live/activities? 
  • Do they have any requests? Needs?
  • Speak with staff to see if the individual has additional needs, such as shoes or clothing, and to gain insight into observed behaviors and/or concerns the individual expresses.
  • While taking notes, always make eye contact and be attentive to the individual.
  • Thank the individual/staff/caregiver for their time.
