Once the employee is stable on the job, after roughly one year, you may transition long-term support to the treatment team and appropriate natural supports.
- Other members of the treatment team, still in contact with the supported employee, can hear about issues on the job.
- If there are difficulties at work or if additional supports are needed the person can still contact you for needed supports/services If they lose their job and/or want a new job the person comes back to your caseload and you offer job development services
- If they lose their job and/or want a new job the person can contact you for job leads or broader job development services
- Your participation in the treatment team is important to ensure effective communication about success on the job or identification of additional supports
